Transitional care is short-term help to get through that stretch: someone at home on discharge day, meals, rides to follow-up appointments, help with bathing while strength comes back, and a set of eyes on how things are going.
If a doctor has ordered home health (nurses or therapists), we don't replace them. We cover the non-medical hours in between.

What we don't do
We're a non-medical agency. Caregivers don't give injections, change wound dressings or handle other nursing tasks. If your family member needs that, we can work alongside a home health or hospice provider.
The easiest way to start is a phone call. Tell us what's going on and we'll tell you honestly whether we can help.
